Piriformis syndrome and sciatica produce remarkably similar symptoms – pain that radiates from the buttock down into the leg – but they have different causes and respond best to different approaches. Treating one when you actually have the other is one of the most common reasons people dealing with buttock and leg pain stay stuck despite doing all the right things. Getting the distinction right is where effective care begins.

What Is Piriformis Syndrome?

The piriformis is a small, deep muscle located in the buttock that runs from the sacrum – the triangular bone at the base of the spine – to the top of the femur. Its job is to help rotate and stabilize the hip. The sciatic nerve runs directly underneath the piriformis in most people, and in roughly 15-20% of the population it actually passes through the muscle itself.

Piriformis syndrome occurs when the piriformis muscle becomes tight, inflamed, or goes into spasm – compressing or irritating the sciatic nerve beneath it. The result is pain, tingling, or numbness that follows the path of the sciatic nerve into the buttock, hip, and down the back of the leg. It can be difficult to distinguish from sciatica originating in the lumbar spine because the nerve being irritated is the same one.

What Is True Sciatica?

True sciatica refers to sciatic nerve irritation that originates in the lumbar spine – most commonly from a herniated or bulging disc pressing on a nerve root, or from spinal stenosis narrowing the canal through which the nerve travels. The nerve is being compressed at its root rather than at the point where it passes through the buttock.

Because the irritation begins higher up in the spine, true sciatica often comes with lower back pain alongside the leg symptoms – though not always. Some people with lumbar disc-related sciatica have minimal back pain and primarily experience leg symptoms, which adds to the diagnostic confusion.

How to Tell the Difference

There’s no single test that definitively distinguishes piriformis syndrome from lumbar-origin sciatica in every case, but several patterns provide strong clues.

Location of primary pain. Piriformis syndrome tends to produce its most intense pain deep in the buttock, often at a specific tender point. The pain may radiate down the leg, but the buttock is usually where it’s most concentrated. Lumbar sciatica often involves more significant lower back involvement, with the leg pain following the nerve’s path from the lower back downward.

What aggravates it. Piriformis syndrome is typically aggravated by sitting for prolonged periods – because sitting compresses the piriformis and its surrounding structures – and often by activities that internally rotate the hip. Climbing stairs, walking uphill, or any movement that stretches or contracts the deep hip rotators can provoke it. Lumbar sciatica is often aggravated by bending forward, prolonged sitting, and movements that increase pressure on the lumbar discs.

Response to straight leg raise. A straight leg raise test – lifting the leg while lying flat – often provokes lumbar sciatica symptoms because it puts tension on the sciatic nerve root at the lumbar level. It’s less consistently provocative in pure piriformis syndrome. Pressing on the piriformis muscle itself, or stretching it through a seated figure-four position, is more likely to reproduce piriformis syndrome symptoms.

Imaging findings. Here’s where things get complicated. Many people with piriformis syndrome have normal lumbar MRI findings – because the compression is happening in the buttock, not the spine. And many people with disc pathology on imaging don’t have symptoms from that pathology. This is why clinical assessment rather than imaging alone is essential for distinguishing between the two.

Why Misdiagnosis Is So Common

The symptoms of piriformis syndrome and lumbar sciatica overlap significantly, and most imaging – which focuses on the spine – won’t show piriformis involvement even when it’s the primary problem. A patient with deep buttock pain and leg symptoms gets an MRI that shows a mild disc bulge, and the disc bulge becomes the assumed culprit even if the actual compression is happening at the piriformis.

Treatment then focuses on the disc – physical therapy exercises designed for lumbar issues, possibly injections targeted at the lumbar spine – none of which addresses the piriformis. The patient doesn’t improve as expected, and no one revisits the diagnosis because the imaging seemed to explain it.

The lesson isn’t that lumbar issues don’t matter – it’s that accurate assessment matters more than fitting the symptoms to the most common diagnosis.

What Drives Piriformis Tightness

Understanding why the piriformis gets tight in the first place is important for addressing the problem rather than just managing the symptoms. Several factors commonly contribute:

Prolonged sitting. Hours of sitting shortens the hip flexors and puts the piriformis in a chronically shortened position. Over time, this creates tightness and trigger points that compress the sciatic nerve.

Hip and pelvic imbalances. Asymmetries in how the pelvis sits and how the hip rotates put uneven load on the piriformis. One side often works harder than the other, which is why piriformis syndrome frequently affects only one side.

Nervous system stress. The piriformis, like all muscles, is regulated by the nervous system. When the nervous system is running a chronic stress response, it keeps the muscles of the hips and lower back in a state of sustained tension – which directly contributes to piriformis tightness and the nerve compression it produces. This is the layer that stretching alone can’t fully address, because the nervous system keeps sending the signal to contract.
